1. First off, start with a new canvas, my sizes are around 740*745, and fill it with #383f44. Next make a new layer and fill it with this pattern, you can use your own, but if you want to use my pattern, you can download it here.

2. Next make a new layer and get your rectangular marquee tool (m) and make a rectangle in the middle of your canvas from top to bottom but about 550 pixels in width, and fill it with white:
Image

3. Now name this layer 'BG2' take out the layer style window of that layer and use the following settings:

4. Next make a new layer and get your rectangular marquee tool (m), now make a selection like the following:
Image

5. Now get your gradient tool, make sure your foreground color is black and just do a small gradient from right to left, do steps 4 and 5 on the other side of the template, and you should have this:
Image

6. Next we are going to work on the header. So make a new layer and call it 'HeaderBG'. Now using your rectangular marquee tool (m) make a selection of how big you want your header to be. Like so:
Image

6. And fill it with white. Now get your gradient tool, make sure your foreground color is black and start making VERY light gradients on your banner, make sure your banner is selected too. Make a gradient from the bottom, and from both sides, try to make it similar to this:
Image

7. Now you may want to add a shadow from the bottom to the top for your banner, use the same way we did with making the side shadows for the template:
Image

8. Now this is where your creativity comes in, add in some text and brushing and you should have a sweet looking banner:
Image

9. If you want to add little switches or ticks like how i did, here's how you can make them. Make a new layer, call it switch1, get your single column marquee tool and make a selection, anywhere it doesn't really matter. Then get your rectangular marquee tool (m) and change it to 'Subtract from selection' and start subtracting from your selection, you may want to zoom in, just leave about 50-70 pixels of selection.
Image

10. Now get your gradient tool, make sure your foreground color is white and make a white to transparent gradient from the top of your selection to the bottom. You should have this:
Image

11. Next duplicate this layer and go to Edit > Transform > Rotate 90 CCW Now move your duplicated version to make it sort of like an upside down 'L' shape and press ctrl+E to merge. Your end product should look like the following:
Image

12. Now back to the template! So we are done with the header, lets begin on the content and navigation bar. So get your rectangular marquee tool (m) and on a new layer, make a selection right under the banner bg and fill it with #383f44. Next type in your text for the buttons and make sure its aligned and spaced out proportionally.
Image

13. Next we are going to make the little outlines, so make a new layer and using your rectangular marquee tool (m) make a selection of a rectangle around a text that you had made earlier to represent your button, like so:
Image

14. Now fill the selection with white, and then still with the selection live, go to select > Modify > Contract A window will come up and put in 1 as the setting. Then press delete after you have press ok for the window. And now you should have a little outline, but that isn't what we wanted, so make another selection.
*Make sure that both left out parts of the white box from your select are equal
*You might want to zoom in on this one too

Image

15. After that delete the selection, and then make another selection, this time horizontally:
Image

16. Now press delete and viola! You should now have your nice little outline, now duplicate this layer and put one for each text that you made. And now we are done with the navigation bar!
Image

17. Next we are going to make the content bars. So get your rectangular marquee tool (m) and on a new layer make a selection and fill it with #383f44 like the following:
Image

18. Now still with the selection, go to Select > Modify > Contract... and set the setting as 5 and press ok. Next press ctrl+shift+I to inverse and on a new layer fill the selection with white. Now go back to the layer before, select it and press ctrl+shift+I. Next click on the second layer and press delete and you should have this after deselecting the selection, a little neat border:
Image

19. Now set the opacity of the white border to 50%. And using the same technique as we did with this small box, make a bigger one with a faded border, like the following:
*You can also add in the little ticks/switches that we made earlier to this too!
Image

20. Finally, we are almost done! We are going to finish it up with the footer. So, make a small selection on the bottom of the template, or the left out space on your BG2 layer.
Image

21. Now press delete, now make a new layer and make another selection, and fill it with white:
img21

To make your bar look like the one in the above, take out the layer style options of the layer and use the following settings:
Image

22. Next just add a shadow from the top of the footer, and you should know the technique by now and you could also add another shadow overlapping it and lowering its opacity to about 5-15%

This concludes our tutorial, you should have something similar to this:
